Department Action

Please approve the Services Agreement and Business Associate Agreement with the Samaritan Inn. The amount of the agreement shall not exceed $21,000 for sick medical visits and prescription assistance for the period March 1, 2016 through February 28, 2017. The guidelines and application for the 2016 Health Care Foundation Service Agreements with the nonprofit agencies to provide health-related services were approved in court on March 14, 2016.

Purchasing Department Action

Request Collin County Health Care Board of Trustees consideration and any action regarding approval of Services Agreement and Business Associate Contract between Collin County Health Care Foundation and Samaritan Inn to provide primary health care services and prescription assistance to citizens who are at or below 100% of the Federal Poverty Level. Agreement shall be effective March 1, 2016 and ends February 28, 2017. The total amount of this agreement shall not exceed $21,000. Further authorize the Health Care Foundation President to finalize and execute same. jd
