This is to request Commissioners Court approval for revision to the One Time Discretionary Unique grant contract #2016-004015. This revision comes at the request of the Texas Department of State Health Services (DSHS), and is meant to "add language to the Statement of Work and Special Provisions". The effective date of this amendment is active May 31, 2016.

Purchasing Department Action

Request Commissioners' Court consideration and any action regarding approval of Amendment #01 to revise the FY2016 CPS/One Time Discretionary Unique Funding Grant, contract #2016-004015. This revision comes at the request of the Texas Department of State Health Services (DSHS), and is meant to "add language to the Statement of Work and Special Provisions". The effective date of this amendment is May 31, 2016. Further authorize the County Judge to finalize and execute same. jd
