Attached is a request for approval of a Business Associate Agreement with Health Services of North Texas to provide WIC supplemental food benefits, nutrition education and counseling to enhance good health care to pregnant and postpartum women, infants and children identified to be at nutritional risk at a remote site of the Wylie Children’s Medical Center, once a week at no charge for space.

I respectfully request approval to partner with Health Services of North Texas to provide services for the families that are unable to travel to the Plano WIC location for WIC services due to transportation. An administrative staff member will be available for services and will be able to work on administrative work when not assisting clients.
