NOTICE OF A REGULAR MEETING OF THE

COLLIN COUNTY HEALTH CARE FOUNDATION

BOARD OF TRUSTEES

Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:

HEALTH CARE FOUNDATION MEETING


1.			Call to order. The board will convene in open session for consideration of the following business matters:

2.			Consent agenda to approve:

	a.	AI-41869 Approved	Disbursements for the period ending August 2, 2016, Auditor.

	b.	AI-41939 Approved	Renewal of the Professional Services Agreement (Agreement No. 08271-11) and approval of the Business Associates Agreement with Jerry Barnett, R. Ph. to provide Pharmacist Services through and including September 30, 2017, Health Care.

	c.	AI-41932 Approved	Amendment No. 8 to the lease agreement with Collin County WIC (Women, Infants and Children) for property located at 900 E. Park Blvd, Plano, Texas to extend the lease term for a period of three (3) years through and including August 31, 2019, Construction & Projects.

	d.	AI-41934 Approved	Ratification of the Board President’s prior approval of an amendment to the contract with the Texas Department of State Health Services (DSHS) for Contract No. 2016-001394-00, Amendment 01: Tuberculosis Prevention and Control-State to accept the increase of grant funds from $152,828 to $305,656 and extend the contract through and including August 31, 2017, Health Care.

	e.	AI-41906 Approved	Filing of the July 18, 2016, Minutes, County Clerk.

	f.	AI-41938 Approved	Receive and File, Monthly Financial Reports for June 2016, Auditor.

			EXECUTIVE SESSION

			Executive Session, in accordance with Chapter 551 of the Government Code.

			The board reserves the right to convene into executive session throughout this meeting.

			Any action resulting from the executive session.
