Department Action

Please approve the amendment to the DSHS Contract #2016-001266-01. The purpose of the amendment is to shift funding from the Equipment Category to other categories in the budget.

When the budget was originally set, funding was allocated for equipment that is no longer necessary to be paid out of grant funds. DSHS contract guidelines require an amendment whenever funds are moved from the equipment category into another category. This change is effective April 15, 2016.

Purchasing Department Action

Request Collin County Health Care Board of Trustees consideration and any action regarding approval of Amendment #01 to revise the DSHS Immunization Contract #2016-001266, to move funding from the Equipment Category to other various categories in the contract. Amendment effective date is April 15, 2016. Further authorize the Board President to finalize and execute same. jd

HR and/or IT Action

Budget Department Action

Amendment to the DSHS Contract No. 2016-001266-01 to move grant funding for equipment in the amount of $7,092 into Travel, Supplies and Other budget categories. Grant funding level is not changing but is being moved to different budget categories. The Other category is increasing by $3,500, Supplies is increasing by $3,052 and Travel by $540.

Auditor's Office Action

Contract amendment to change the budget to move $7,092 out of the Equipment category to be split among Travel, Supplies, and Other. This is not a decrease in funding but only a change in category budgets. Budget amendment to follow.
