Attached is the FY2016-2017 Interlocal Application for DSHS Immunizations Program funds for the one-year grant period 9/1/2015-8/31/2017 in the amount of $354,062 which is level funding for the upcoming fiscal year.  Due to the grant application deadline being 4/29/2016, the application was submitted to DSHS in a timely manner and needs ratification. The purpose of the Immunizations Program is to support local health departments in their efforts to provide immunization services to their communities as well as enhance epidemiology response to vaccine preventable diseases such as Perinatal Hepatitis B. The Immunizations Program also educates, manages, and assists approximately 70 healthcare providers who participate in the Texas Vaccines for Children (TVFC) Program to ensure they understand the proper storage and maintenance of state-issued vaccines. Collin County has an admirable history which spans over 25 years of providing the residents of its community with immunization services. Immunizations are also required by the Texas Administrative Code §97.63 for children attending schools in Texas. The Immunizations grant funds will be used towards the salary and fringe benefits of twelve department staff members who work in the Immunizations and Epidemiology departments. By applying for and receiving state funding, Collin County Health Care Services (CCHCS) will be able to continue to be a safety net provider of affordable and necessary immunizations for Collin County residents and their families.

Grant term is September 1, 2015 through August 31, 2017.  No County match is required. jd
