Department Action

Attached is an amendment to the existing Vaccination Capacity grant contract with DSHS.

The purpose of this amendment is to extend the contract term, revise the statement of work, and revise the grant budget by redistributing existing funds among the budget categories without changing the total contract amount. This amendment would extend the contract an additional year and the new contract end date would be on 06/30/2025.

I respectfully request approval of this amendment.

Request Health Care Foundation consideration and any action regarding approval to accept Amendment No. 2 for Texas Department of State Health Services (DSHS) Vaccination Capacity grant, extend the term to June 30, 2025, and authorize Board President to execute same. gz

Auditor's Office Action

Grant contract amendment to extend the grant end date from June 30, 2024 to June 30, 2025. No additional funding is being awarded, so total contract amounts are still $1,995,432 for project 1 and $910,714 for project 2. Original grant review committee form is attached.
