Attached is the approved two-year no-cost extension amendment to the COVID-19 Health Disparities grant. The current contract term is from September 27, 2021 through May 31, 2024, and this will extend the term to end on May 31, 2026. This approved extension will allow Collin County Health Care Services to continue to enhance the Health Department’s capacity to strengthen partnerships with local stakeholders and information sharing to promote health in all communities.
Health Care Services’ department funds may be utilized for expenses that are not approved for purchase on the grant but are critical to department operations, including, but not limited to, supplies, uniforms, or training required for all department employees. It is estimated that approximately $200 per employee will be utilized from the department’s existing budget.
Health Care Services will monitor the funding closely in partnership with the Collin County Auditor’s Office to ensure this grant will not be overspent. One position (1 PHEP Planner) will be terminated at the time funding runs out, which will be before the grant’s end date of May 31, 2026.
Local Health Department awards are “one-time” only and activities should not incur future reoccurring costs beyond May 31, 2026.

No increase to headcount. The estimated personnel cost for salary and fringe, to cover this position at 100%, is $217,661.00 per the grant benefits calculator. Per Auditor’s comment, no extra funds are awarded with this contract extension and the remaining grant budget is $74,814.04.
