Department Action

Attached is an amendment for the FY 2024-2025 LPHS/RLSS grant. The current contract term and funding amount will not change. This amendment will not incur additional costs to the county. This amendment was requested by DSHS due to revisions to the Statement of Work (SOW). It also includes revisions to the final reporting due dates, updates to the Terms and Conditions, and updates to the Contract Affirmations sections of the contract.

Request Health Care Foundation's consideration and any action regarding approval of amendment for Texas Department of State Health Services (DSHS) the Local Public Health Services/Regional Local Services System (LPHS/RLSS) grant to update the statement of work, federal and state funding splits, terms and conditions, contract affirmations, and final report due dates, and authorize Board President to execute same.
