Collin County Health Care Services requests Commissioners Court acceptance of an amendment to the Public Health Infrastructure grant contract from the Texas Department of State Health Services (DSHS). This amendment incorporates revisions to the Statement of Work and updates to the Contract Affirmations and Uniform Terms and Conditions sections.
In addition, attached is a DSHS-approved budget that moves funds between categories to support the (2) Financial Analyst (Position IDs: 300516, 300526), (2) Functional Analyst (Position IDs: 300621, 300517) , and (1) Health Care Analyst (Position ID: 300528) budgeted on this grant. The (1) vacant Health Care Analyst (Position ID: 300624) position will be discontinued to re-allocate funding towards the existing positions listed.
Health Care requests approval of this amendment and the use of existing Health Care department funds to support the personnel on this grant. Existing Health Care funds will be used to purchase items for staff that are critical for department operations but are not allowable to be purchased with grant funds, which are an estimated $500/year per staff. |

Amendment No. 1 to the Public Health Infrastructure Grant Award (HHS001311000001) for the grant period April 24, 2023 through November 30, 2027, proposes to amend the statement of work, contract affirmations, and uniform terms and conditions. Contract also amends budget to move funds from other categories to the personnel category. The estimated personnel cost for salary and fringe to support the existing (2) Financial Analysts (Position IDs: 300516, 300526), (2) Functional Analysts (Position IDs: 300621, 300517), and (1) Health Care Analyst (Position ID: 300527) at 100%, is $1,683,340.62 per the grant benefits calculator. The estimated grant amount for these positions is $1,625,736 which will create a shortfall of $57,604.62. A county match to supplement grant funding for the full cost of these positions may be required. |
