Request Commissioners Court consideration and any action regarding approval of Amendment No. 1 to contract 2018-340, Insurance, Life and Accidental Death and Dismemberment (AD&D) with Gary Insurance Services, Inc. to: (1) increase rate per the attached documentation; (2) lock-in rate for the next two renewals; and (3) renew contract for a 1-year period of January 1, 2022 through and including December 31, 2022, and authorize Purchasing Agent to execute same. gz
