Department Action

Please approve the Service Agreement and Business Associate Agreement with Allen Community Outreach. The amount of the agreement shall not exceed $1,000 for prescription assistance, sick medical visits, laboratory, and x-ray services for the period March 1, 2023 through February 29, 2024. The guidelines and application for the 2023 Health Care Foundation Service Agreements with the nonprofit agencies to provide health-related services were approved by the Health Care Foundation Board of Trustees on February 6, 2023.
