REQUESTING THE POSTMORTEM TOXICOLOGY TESTING CONTRACT WITH NATIONAL MEDICAL SERVICES, INC DBA NMS LABS FOR THE MEDICAL EXAMINERS OFFICE BE EXTENDED FOR THE PERIOD OF MAY 18, 2023 THROUGH MAY 17, 2024. REPLACING LINE ITEMS AND PRICING; AND CHANGE PRICING TO PROSED UNIT PRICING.

Request Commissioners Court consideration and any action regarding approval of Change Order No. 5 to Contract No. 2020-140, Testing: Postmortem Toxicology with National Medical Services, Inc. dba NMS Labs to: (1) renew contract for a 1-year period of May 18, 2023 through and including May 17, 2024; (2) include a price redetermination per section 4.6 of the contract per attached documentation; (3) replace Exhibit A with Exhibit B, per the attached documentation, with updated line items and prices; and further allow Purchasing Agent to execute same.
