Attached is the FY2025 Interlocal Application for DSHS Immunizations Program funds for the one-year grant period 9/1/2024-8/31/2025 for $354,062. This award amount is equivalent to the prior FY2024 award amount. The purpose of the Immunizations Program is to support local health departments in their efforts to provide immunization services to their communities as well as enhance epidemiology response to vaccine preventable diseases such as Perinatal Hepatitis B.
This is a routine grant renewal with staff funded by a combination of grant and Collin County funds. Grant funding is primarily used for staff salary and fringe benefits to maximize the utilization of these funds. Collin County funds the remainder of salary and fringe benefits as well as items such as cell phone service plans, internet hotspot service plans, training, travel, office supplies, uniforms, software, equipment replacements as needed, and other items necessary to perform job duties. It is estimated that Training & Travel and Maintenance & Operations items combined cost approximately $2000 on average per employee per year on this grant, which have been budgeted in Health Care Services’ current department budget. Computer, cell phone, and other electronic item replacements for the positions on this grant have historically been performed by Collin County IT on schedule with the rest of the department and will continue to do so.
Collin County Health Care Services requests approval of this grant renewal application and the use of existing Health Care department funds to support the personnel on this grant. Health Care funds will be used for travel, trainings, and supplies to continue to meet program objectives.

Per the attached budget, the Immunization Locals ILA Renewal FY 2025 will fund 12 existing positions and personnel at the percentage of FTE indicated on the budget. Based on this percentage and current salaries, the estimated total for salary and fringe for these 12 personnel is $352,266.60. The amount estimated on the attached grant summary budget for these twelve positions is $352,562. A county match to supplement grant funding for the full cost of these positions may be required. Position numbers attached to this grant are 200936, 200950, 201264, 201271, 201274, 201467, 201475, 201770, 00300654, 00300456, 00300214, and 00300650.
