Please approve the Services Agreement and Business Associate Agreement with Community Health Service Agency, Inc. The amount of the agreement shall not exceed $20,000 for sick medical visits and prescription assistance for the period March 1, 2021 through February 28, 2022. The guidelines and application for the 2021 Health Care Foundation Service Agreements with the nonprofit agencies to provide health-related services were approved in court on February 8, 2021.
