Attached is the FY 2021-2025 application for DSHS DIS Workforce Grant funds for a grant period that will begin upon contract execution through December 31, 2025. The total amount for Contract Term 1: January 1, 2022 – August 31, 2022 for this grant award is $469,545.00. Due to the grant application being due 11/04/2021, the application was submitted to DSHS in a timely manner and needs approval. I respectfully request that you approve the application for this new funding.

FY 2021–2025 DSHS Disease Intervention Specialist (DIS) Workforce Grant in the amount of $469,545.00 in state funds. Grant funds to cover 100% salary and benefits for five (5) new personnel, programming, supplies, training and travel as well as other related expenses and duties. Contract period is January 1, 2022 through August 31, 2022.

Grant award has been reviewed by the grant review committee as per attached. The DIS grant is meant to support disease intervention activities in Collin County such as disease surveillance, infection control, to also include responding to COVID-19.Grant in the amount of $469,545 with no county match required. Grant funds 5 positions at 100% plus equipment and supplies. Grant period is January 1, 2022 through August 31, 2022.
