Department Action

Attached is the approved one-year extension amendment to the COVID-19 Health Disparities grant. The current contract term is from September 27, 2021 through May 31, 2023, and this will extend the term to end on May 31, 2024. This approved extension will allow Collin County Health Care Services to continue to enhance the Health Department’s capacity to strengthen partnerships with local stakeholders and information sharing to promote health in all communities.

Health Care Services will monitor the funding closely in partnership with the Collin County Auditor’s Office to ensure this grant will not be overspent. These 3 positions (2 PHEP Planners and 1 Community Health Specialist) will be terminated at the time funding runs out, which will be before the grant’s end date of May 31, 2024.

Request Health Care Foundation Board consideration and any action regarding approval to extend COVID-19 grant to May 31, 2024 with no additional funding and authorize Board President to execute same. gz

HR and/or IT Action

Amendment No. 1 to COVID-19 Health Disparities Grant Program to extend one (1) current Community Health Specialist position for 20 months (filled), and two (2) current PHEP Planner positions for 23 months (1 vacant and 1 filled) through May 31, 2024. No increase to headcount. Grant is meant to fund these positions at 100%. Using the grant benefits calculator to estimate the cost of salary and fringe for these 3 positions, the total estimated cost is higher than the amount budgeted on the Budget Detail Form. Cost for salary and fringe budgeted is $447,035, calculator estimates $495,845.

Budget Department Action

Amendment No. 1 to Contract No. HHS001057600012 COVID-19 Health Disparities Grant Program for a one-year extension. No additional funding has been awarded with this extension. Amendment to extend three (3) positions (no increase to headcount) funded at 100% as well as operating costs. Insufficient grant funds to cover entirety of extension for all three (3) positions. Positions to be terminated when grant funds run out. Grant period through May 31, 2024. (2108)-HC Grants Fund.

Auditor's Office Action

Grant contract extension to extend contract dates. Grant dates extended to end on May 31, 2024. No additional funding awarded to extend three existing positions. There will not be sufficient funding to fund all three positions beyond November 2023.

Commissioners Court
