Department Action

Attached is the FY 2021-2023 application for DSHS COVID-19 Health Disparities funds for a grant period that will begin upon contract execution through May 31, 2023. Total funds for this grant are $500,000.00. Due to the grant application being due 07/23/2021, the application was submitted to DSHS in a timely manner and needs approval. This grant is a unique opportunity to expand the public health infrastructure that will serve all Collin County residents impacted by COVID-19 and other public health threats.

FY2021-2023 COVID-19 Health Disparities Grant in the amount of $500,000 with no county match required. Grant includes three (3) personnel, training and travel, supplies, computer equipment and wireless service devices. Grant term effective upon execution through May 31, 2023.

Grant application has been reviewed by the grant review committee as per attached. Grant funding for the COVID-19 Healthcare Disparities Grant in the amount of $500,000 with no county match required. Grant funds 3 positions at 100% plus equipment. Grant period is upon execution through 5/31/2023.
