Consideration and approval of Amendment No. 6 to Contract 07335-10 with Dr. James Shupe for psychological services for the Probate Court in the amount of $72,000. The request is to extend the contract effective October 1, 2016 through and including September 30, 2017 at the same terms and conditions. Fees for Dr. Shupe's services will not increase with this amendment.

Purchasing Department Action

Request Commissioners' Court consideration and any action for approval of Amendment #6 to Physician's Services Agreement with Dr. James Shupe, Contract 07335-10 to extend contract period for additional year and allow Purchasing Agent to execute same. There is no cost increase associated with this renewal. jt
