Department Action

Consideration and approval of an amendment to Contract No. 537-16-0038-00001, with the Texas Department of State Health Services for access to online Vital Records.

The Agreement allows the County Clerk access to search and issue vital records to qualified applicants.

Purchasing Department Action

Request Commissioners' Court consideration and any action regarding an Amendment No. 1 to Contract No. 537-16-0038-00001 with Texas Department of State Health Services (DSHS) for access to online Vital Records and authorize County Judge to execute same. Amendment will extend the agreement until August 31, 2018, add Data Use Agreement and other changes as detailed in the attached amendment.

Attorney has reviewed the proposed amendment.

Budget Department Action

Amendment No. 1 to Contract No. 537-16-0038-00001 with Texas Department of State Health Services (DSHS) for access to online Vital Records. Amendment to extend the contract effective September 01, 2015 through August 31, 2018, add Data Use Agreement and other changes as detailed in the attached amendment. Budgeted funds available in County Clerk’s-0801 budget to cover the cost of DSHS access to on-line Vital Records.
