I respectfully request the Court to consider and approve Amendment #4 to Interlocal Agreement with City of Lowry Crossing for Collin County to provide EMS services through American Medical Response (AMR).

Request Commissioners' Court consideration and any action regarding approval of Amendment 4 to the Interlocal EMS Services Agreement with the City of Lowry Crossing, contract 2014-286 to extend the contract for one (1) year beginning November 1, 2017 through and including October 31, 2018, the FY2018 annual amount $22,169.50 in quarterly payments, further authorize the Purchasing Agent to finalize and execute same. go
