Request extension of Testing: Postmortem toxicology contract IFB NO. 2015-286 for a one year period from October 1, 2018 through and including September 30, 2019. Pricing will have an overall increase of up to 3%.

Request Commissioners' Court consideration and action regarding approval of change order No. 3 to contract 2015-286, Testing: Post Mortem Toxicology with National Medical Services, Inc. dba NMS Labs to 1) include a price redetermination per section 4.5 of the contract, (see attached) for an overall increase of 3% and 2) to extend the contract for the period October 1, 2018 continuing through and including September 30, 2019 and further allow purchasing agent to execute the same. sh
