 This is to request Commissioners Court acceptance for the amendment to the FY 2020 Public Health Emergency Preparedness (PHEP) Cities Readiness Initiative (CRI) contract with the Texas Department of State Health Services (DSHS). 
 
DSHS recently provided a limited amount of additional grant funding for various contracts and has issued a contract amendment to formally implement those changes.  Attached for approval is the amendment to the FY 2020 CRI grant (537-18-0141-00001) for the one year grant period 7/1/2019-6/30/2020.  The initial funding amount of $128,650 was increased by $4,117 for a total of $132,767.  A 10% match is required for all funds spent.  We expect to meet that obligation through in-kind salary and fringe match from existing Collin County staff as well as MRC volunteer hours.
 
The purpose of the CRI grant is to enhance preparedness efforts in our county and improve our ability to respond to public health emergencies, especially those requiring life-saving medication and/or medical supplies.  Program funds will be used towards existing CRI personnel costs as well as purchasing related program supplies.

 Request Health Care Foundation approval of Amendment No. 3 to the FY2020 Public Health Emergency Preparedness (PHEP) Cities Readiness (CRI) Grant form the Texas Department of State Health Services (DSHS), Contract No. 537-18-0141-00001 to amend the Budget per the attached and authorize Board President to execute same. sh

Original application reviewed by the grant review committee as per attached. Award has been increased as per attached amendment contract. Federal funds increase in the amount of $4,117 and matching funds increase in the amount of $412. Matching funds can be in kind.
