Request Collin County Health Care Trust Foundation consideration and approval of amendment No. 5 to the lease agreement between Collin County Health Care Trust Foundation and Collin County WIC Program for property located at 825 N. McDonald, McKinney, Texas.  

1.     Effective January 1, 2020, extend the lease for a five year period, to expire December 31, 2024.

2.     Effective January 1, 2020, decrease lease floor area 653 square foot leaving a total of 2,184.5 square feet. Base rent will remain at $16.00 per square foot. $34,952.00 per year payable in monthly installments of $2,912.67 per month in advance.

McDonald, McKinney, Texas to 1)  extend the lease for a five (5) year period beginning January 1, 2020 continuing through and including December 31, 2024 and 2) decrease lease floor area 653 square foot leaving a total of 2,184.5 square feet. Base rent will remain at $16.00 per square foot. $34,952.00 per year payable in monthly installments of $2,912.67 per month in advance. Further authorize the Board President to execute same. MD
