Request Collin County Health Care Trust Foundation consideration and approval of Amendment No. 8 to the Lease Agreement between Collin County Health Care Trust Foundation and Collin County WIC for property located at 900 E. Park Blvd., Plano, TX. Amendment No. 8 will extend the lease term for a period of three years commencing September 1, 2016 through August 31, 2019. Current lease is for 5,384 square foot at $15.00 per square foot payable in monthly installments of $6,730.00.

Request Collin County Health Care Trust Foundation's consideration and any action regarding approval of Amendment No. 8 to the Lease Agreement between Collin County Health Care Trust Foundation and Collin County WIC for property located at 900 E. Park Blvd., Plano, TX, to extend the lease term for a period of three years commencing September 1, 2016 and continuing through and including August 31, 2019. Further authorize the Board President to execute same. md
