Department Action

For the upcoming 2016-2017 fiscal year, I respectfully request the renewal of the professional service agreement with Collin County for Jerry Barnett, R. Ph..The cost of Jerry Barnett service agreement is a budgeted expense that would not require any additional county funding. Jerry Barnett provides services as a pharmacist for Collin County Health Care Services (CCHCS). Because CCHCS maintains a Class D pharmacy, we are required to have a pharmacist perform various functions in order to meet our statutory requirements for our various clinics.

Purchasing Department Action

Request Health Care Foundation consideration and any action regarding the approval to renew Agreement No. 08271-11, Professional Services, Pharmacist with Jerry Barnett R. Ph. beginning October 1, 2016 through September 30, 2017. Further authorize the Board President to finalize the attached Business Associate Agreement. There is no price increase associated with this renewal. jd

Budget Department Action

Request to renew the Professional Service Agreement with Jerry Barnett, R. Ph.. Budgeted funds available in the FY17 State TB Grant Fund (108). Contract amount as well as terms and conditions remain the same and new contract term is October 1, 2016 through September 30, 2017.

Auditor's Office Action

Grant funds available in the amount of $2,400 for the reimbursement of this agreement.
