This is to request Commissioners Court acceptance for the FY 2017 Immunizations Program grant award.

The FY 2017 award is being distributed as an amendment to the existing contract, increasing the DSHS grant funds for an additional $354,062 and extending the contract through August 31, 2017 which is level funding.

The FY 2016 contract, as well as the FY 2017 amendment, are included for reference.

The purpose of the Immunizations Program is to support local health departments in their efforts to provide immunization services to their communities as well as enhance epidemiology response to vaccine preventable diseases such as Perinatal Hepatitis B.  The Immunizations Program also educates, manages, and assists approximately 70 healthcare providers who participate in the Texas Vaccines for Children (TVFC) Program to ensure they understand the proper storage and maintenance of state-issued vaccines.  Collin County has an admirable history which spans over 25 years of providing the residents of its community with immunization services.  Immunizations are also required by the Texas Administrative Code §97.63 for children attending schools in Texas.  The Immunizations grant funds will be used towards the salary and fringe benefits of twelve department staff members who work in the Immunizations and Epidemiology departments.  By applying for and receiving state funding, Collin County Health Care Services (CCHCS) will be able to continue to be a safety net provider of affordable and necessary immunizations for Collin County residents and their families.
Purchasing Department Action
Request Collin County Health Care Board of Trustees consideration and any action regarding approval of acceptance of the FY2017 Immunization Program Grant award distributed as an amendment to the existing FY2016 contract through the Texas State Department of State Health Services to increase grant funds and extend the contract through August 31, 2017, Contract No. 2016-001266-00. Further authorize the Board President to finalize and execute same. jd

FY 2017 Immunization Program to provide immunization services to children and adults at or below the poverty level. Grant funding in the amount of $354,062 in federal funds to partially fund 12 employees salaries and benefits, travel, supplies and postage. Grant period is 9/1/2015 through 08/31/2017. No county match is required.
