Department Action
 Attached is the FY 2021-2023/2024 application for DSHS COVID-19 Vaccine Supplemental funds for a grant period that will begin upon contract execution through June 30, 2024.
 
Total funds for this grant are $2,906,146.00, which is split between two projects.
 
The Project 1 award of $1,995,432.00 is provided for Collin County to increase COVID-19 vaccination capacity, including among high-risk and underserved populations. 
 
The Project 3 award of $910,714.00 is to ensure equitable distribution and administration of COVID-19 vaccines to Collin County residents.
 
The Centers for Disease Control and Prevention awarded Texas funding for COVID-19 vaccination implementation activities and data analysis.  Local Health Department awards are “one-time” only and activities should not incur future reoccurring costs beyond June 30, 2024.

DSHS COVID-19 Vaccination Capacity Contract No. HHS001019500012 in the amount of $2,906,146.00. Project 1 in the amount of $1,995,432.00 will cover salary and benefits for surge team personnel as well as travel, equipment, supplies and a vaccine freezer. Project 3 in the amount of $910,714.00 will cover salary and benefits for additional surge team personnel as well as travel, supplies and other program costs. Grant term effective upon execution through June 30, 2024.

 
Auditor's Office Action
Grant award has been reviewed by the grant review committee as per attached. COVID-19 vaccine activities. Create a surge team that will go into the community and vaccinate homebound patients. Increase surge capacity for back to school COVID-19 vaccine efforts as well as COVID-19 booster efforts. Total Contract for Project 1-is $1,995,432.00 and Project 3 is $910,714. Grant period is upon contract execution through June 30, 2024.
