Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:

FY22 TB Federal Grant Application

Renewal grant for $114,386 (Level Funding) from the Texas Department of State Health Services for Tuberculosis (TB) services. The county's match as requried by the contract is $22,877. Both the awarded renewal grant funds and the county's match funds will be used toward the existing salaries of several TB Clinic staff members as part of the effort to provide TB services for the community. The performance measures included in the contract are directed towards the TB Program's ability to provide evaluate and treat individuals who are exposed or infected with the TB germ or have active TB disease. The TB Clinic manages the care of these infected or ill patients from 6 months up to 2 years during the course of their treatment in order to ensure they successfully complete their treatment.

FY 2022 DSHS Federal Grant for TB Prevention and Control in the amount of $114,386 with an in kind match of $22,877 to be met with existing employee salaries and benefits. Grant includes personnel, training and travel, supplies, contract services with Prima Care, language line, patient transport and wireless service. Grant term is January 1, 2022 through December 31, 2022. (2108)-HealthCare Grant Fund.

Application for federal grant funding for the TB program in the amount of $114,386 with a county match of $22,877 required. County match can be in-kind. Grant funds 2 positions at 49% grant funded, with the 51% being county funded. Grant period is 1/1/2022 through 12/31/2022.
