Attached is the Fiscal Year (FY) 2026 Immunizations renewal grant application. This application covers one year from September 1, 2025 to August 31, 2026, with level funding set at $354,062. These funds are essential for supporting Collin County’s immunizations program that provides immunization services to the community, as well as enhance epidemiology response to vaccine preventable diseases such as Perinatal Hepatitis B.
Historically, Collin County Health Care Services has allocated this grant funding to partially support four Registered Nurses (Position ID: 201271, 201274, 201475, 300770), one Licensed Vocational Nurse (LVN) (Position ID: 201264), three Tech I positions (Position ID: 300654, 300650, 201467), one Immunization Service Aid (Position ID: 200950), one Community Health Specialist (Position ID: 300456), one Health Care Analyst (Position ID: 300214), and one Assistant Public Health Director (Position ID: 300816). The following positions have been included in the CCHCS FY26 budget and will be 100% funded by the Collin County Health Care Foundation: Registered Nurse (201271, 300770), LVN (201264), Tech I (300654, 300650), Immunization Service Aid (200950), and Assistant Public Health Director (300816). The movement of these positions to 100% Collin County funding will be offset by increasing the grant funding percentages of the remaining staff. This will result in net neutral expenses with no new funding required by Collin County.
The current FY26 Immunizations budget, excluding indirect costs, will fund 5.05 full-time equivalents (FTEs) for the following positions: 201274 (85%), 201475 (100%), 300456 (100%), 300214 (100%), 200796 (10%), 300285 (10%), and 201467 (100%). The percentages indicate the portion of each employee’s time funded by the grant, with the remaining funding provided by Collin County or another grant. Three positions (Position IDs: 201475, 300214, 201467) historically funded by the Immunizations grant will face funding shortfalls in FY26 due to the Collin County requirement to add a 10% de minimis indirect cost rate to grants in FY26. A separate request for funding for these positions has been included in Health Care Services’ FY26 budget, which the Commissioners Court will review during the regular budget process. If the FY26 budget requests are not approved, the positions will be laid off once available grant funding is exhausted.
Collin County Health Care Services requests approval of this grant renewal application and the use of existing Health Care department funds to support department-required items that are not allowable expenses on the grant. Health Care department funds have historically been used for this purpose and no additional funds are requested.

Grant request funds seven existing healthcare positions: four (300456 12-mos, 300214 1-mo, 201475 2-mos, 201467 6-mos - no increase to headcount) estimated at 100%, two (200796 12-mos, 300285 12-mos - no increase to headcount) estimated at 10%, and one (201274 12-mos - no increase to headcount) estimated at 85% for varying periods. The $310,749 grant budget covers $306,029 of the estimated $591,998 total salary and benefits. The county will contribute an estimated $285,969 to cover the remaining costs. The grant also funds training, travel, supplies, and other program costs. However, partially funded positions (300214 1-mo, 201475 2-mos, and 201467 6-mos) will require additional funding in FY26, and their inclusion in the FY26 Healthcare Budget is requested.
